The Prescription Drug Program
You do not need claim forms when visiting a participating
pharmacy. Simply present your ID card and your prescription
to the pharmacist. When you present a prescription
to a pharmacy to be filled under the terms of this Plan, that
request is not considered a claim under these procedures.
However, if your request for a prescription is denied
in whole or in part, you may file a claim under these
procedures.
Also, if you obtain a prescription from an Out-of-Network
pharmacy and you wish to request reimbursement, you
will require a claim form. See here.
