Assignment of Benefits
You cannot assign or transfer benefits in any manner or
to any extent to anyone other than a health services
provider (which you do by completing a form provided
by the health services provider or the Plan). You cannot
pledge the benefits owed to you for the purpose of
obtaining a loan.
